Our team offers gentle reminders and organization tools to help residents take their prescribed medications on schedule, supporting independence and overall wellness.

We work closely with licensed caseworkers to ensure care plans are tailored to each individual, maintaining consistent communication and oversight.

Residents enjoy three nutritious meals every day along with weekly housekeeping, ensuring both comfort and cleanliness at home.

The Red Fern Haven Admission Journey

​Complete the Intake Form

Begin the admission process by filling out our intake form, available on our website. This step gives us important background information about the resident’s needs and preferences, helping us start with a clear understanding of their unique situation.

Assessment and Document Collection

After receiving the form, our team reviews the information and begins an assessment. At this stage, we collect essential documents and details to help determine how we can best support the resident. This ensures we’re prepared to meet individual needs from the very beginning.

Eligibility Review and Pre-Approval

Once all paperwork is gathered, our admissions team evaluates eligibility. We carefully confirm that Red Fern Haven is the right fit to provide the level of care needed, building trust and clarity for both the resident and their family

Facility Tour
If eligibility criteria are met, we invite the resident and their family or case manager to tour Red Fern Haven. This visit is an opportunity to meet our team, experience our welcoming atmosphere, and see the living spaces and services that set us apart.
5.
Move-In Decision and Scheduling
When the resident decides to join our community, we work together to set a move-in date that is convenient. This stage includes orientation for both the resident and family, covering daily routines, available services, and introductions to our supportive staff.
6.
Personalized Care Plan Development

After orientation, we design an individualized care plan tailored to the resident’s goals and preferences. This plan is created in partnership with the resident, their loved ones, and case managers, ensuring it reflects their unique needs and promotes independence.

Ongoing Support and Quality Care

Once the resident is settled in, our commitment continues with consistent follow-up and adaptable support. From medication reminders to assistance with daily living and coordination with outside healthcare providers, our focus is always on helping residents thrive in a safe, compassionate community.
